The COVID-19 pandemic has completely changed the way we take care of our sick and those in need. In cases where measures such as a quarantine make it impossible for us to attend to our patient in person, remote monitoring becomes essential. But how can we check vital signs, provide assistance and moral support if we can’t be there with them in person? Thanks to recent advancements, scientists, physicians, and engineers can finally provide various situations.

In this review we will get to the heart of the MoviWear ecosystem, examining the first in a series of additional sensors. These additional sensors allow the Safety Alert Pendant’s users to monitor vital parameters in real time, making it possible for doctors and medical staff to provide remote assistance 24/7.

MoviWear & Cosinuss°: two “heavenly gifts” to save lives

We have had the pleasure to work with Moviwear, a Canadian company, founded by Jacob Moshinsky. Moviwear created a healthcare new-generation Remote Patient Monitoring platform which provides users with a 24/7 virtual monitoring through several wearable devices, which are interrelated and connected to cloud-based (MovieWearMed) and APP platforms (Nurture Watch App). In my previous review on the Safety Alert Pendant I illustrated the main device which, worn as a pendant by a patient, allows constant communication with family and medical staff through its SIM module and GPS providing 24/7 availability.

The German Cosinuss°, based in Munich, is a certified medical technology company specialized in the remote monitoring of vital parameters in real time. Among the numerous patents mostly addressed to professional athletes, the team creates medically certified wireless in-ear sensors able to continuously detect all the most relevant vitals with the highest precision. Born in 2011, Cosinuss° pioneers the field of remote and mobile monitoring.

Cosinuss° One: the device

What does “class 1m medical device” mean? These devices do not need to be introduced into the body cavity, making them non-invasive. Approved by the accredited Notified Body TÜV, Cosinuss° One follows the regulatory requirements specified by the European Directives for Medical Devices (MDD, MDR) through the harmonized standard EN ISO 13485:2016. In Europe the product is certified and approved by the Department of Health.

The ergonomic shape of Cosinuss° One makes it comfortable to wear. The water resistance property allows the device to be used during daily workout, intense training sessions or in cases of high fever or other conditions causing sweat. Essential accurate vital sign measurements suit all types of situations, from professional athletes to hospitalized patients of all ages.

Cosinuss° One combines an award-winning design with great ease of use and cutting edge technological performances. Thanks to its small size (4.5 cm x 1.8 cm x 3.8 cm) and weight (6,5 g) and to a 10h battery lifetime (with a Micro-USB type charging cable), users wear it continuously while performing their usual tasks. Since data transmission is via Bluetooth 4.0 or ANT+ on a 2,4GHz radio frequency, all devices using this technology can connect to Cosinuss° One.

MoviWearMed: The Remote Patient Monitoring Platform

Data sharing via Bluetooth Technology is key to understanding how a remote patient monitoring platform of vital parameters is able to work. Central to the Moviwear ecosystem is the Safety Alert Pendant device – see my previous review here and the original article in Italian.

First in a series of sensors connecting to the Safety Alert Pendant, Cosinuss° One allows users to monitor their body temperature directly via the Pendant itself. Thanks to the Bluetooth connectivity the Pendant connects directly to Cosinuss° One. The Safety Alert Pendant is the core which groups together responses coming from a variety of sensors and then forwards them directly on MoviWear servers and on the MoviWearMed cloud via its own 4G connection.

The cloud MoviWearMed (https://medical.nurturewatch.online) requires the same log-in information which users already possess and use for the NurtureWatch App. Once logged in, users will access the monitoring of all vital parameters and data automatically gathered by the Safety Alert Pendant.

MoviWearMed Home screen is easy-to-use and complete. In just a few seconds, users can access the patient’s position (GPS location) and the following parameters:

  • Heart rate
  • Body temperature
  • Respiratory rate
  • Glucose
  • Blood pressure (Sys)
  • Pedometer
  • Physical position of a patient (i.e. standing or sitting)
  • Average RRi
  • Sp02 (oxygen hemoglobin saturation)
  • rMSSD
  • ECG (electrocardiogram).

A simple and user-friendly interface displays a 12 tiles checkerboard with icons marking each parameter and showing the measured values. By simply clicking a tile, users will access the corresponding diagram or sub-menu to monitor the value history. The Safety Alert Pendant is created to follow users/patients everywhere therefore, all data collected by the external sensors is continuously registered for further examination. To increase accuracy, users can set thresholds. An alert is emitted when a threshold is exceeded. Unlike the central button on the Safety Alert Pendant which is a manual generic alarm-button for the users’ convenience, the other sensors work autonomously. This is crucial in case patients are unconscious.

Finally, to enter personal data or annotations there’s a specific screen where it’s possible to register additional users/patients to the same Pendant – a useful feature in case of an elderly couple living together. A drop-down menu will show different values. A special view filter allows to sort the users according to their health conditions with priority given to those more at risk.

Cosinuss° One: the acquisition of parameters

Cosinuss° detects just a few of the parameters listed on MoviWearMed homepage. Having been created for the remote patient monitoring of body temperature through the ears, this one remains the main function of the device, together with the measuring of heart rate variability (rMSSD) and average RR intervals (RRi). Using it is simple and can be run on the MoviWearMed platform, as well as on the NurtureWatch app, by clicking the green button on the webpage and choosing the sensor to launch and update its rate. As regular analysis is crucial, users can set how long a sensor shall register vitals and how often it measures and transfers them to the platform. There are two specific drop-down menus: the first marks the analysis span – indefinitely or for the following 30 seconds, 1 minute, 3, 5, 15, 30 up to a maximum of 1 hour (i.e. 60 minutes). On the second menu users can choose how often the sensor shall collect measurements and set the “online” frequency – every second, all 10 seconds, 30, every minute, all 5 minutes, up to a maximum of once every 10 minutes.

To help understand the full potential of Cosinuss° One, in my test, I chose indefinite monitoring with online frequency, to give an idea of the accuracy of the collected data. The rich variety of parameters shown in the diagrams, and the great simplicity of the MoviWearMed platform, made me feel almost like a doctor! In my test I started collecting data before wearing Cosinuss° One so as to understand how long after wearing it the device starts to measure the body temperature; I also voluntarily altered my heartbeat by breathing heavily or by holding my breath.

Closing remarks

MoviWear is engaged to make the difference in the evolution of healthcare through information, technology, simplified processes, and data management. The integration with wearable devices addressed to elderly people or patients now makes specific emergency actions simplified and far more effective, thanks to programmable alerts sent directly to family members, doctors and medical staff. Integrated into the MoviWearMed platform through its connection with the Safety Alert Pendant, Cosinuss° One collects and registers vital parameters on the cloud, thus allowing instant viewing. Given the increasing usage of remote healthcare in order to help prevent COVID-19-related risks, this seems to be a winning solution.

Jacob Moshinsky’s innovative approach to digital healthcare coupled with the Cosinuss° team’s accomplishments make remote monitoring available for everyone. Now, we are able to protect our loved ones, know thant they are home safe – avoiding risks of contagion, for example.

This way, assistance is always continuous and we are able to stay updated on our loved one’s health. The medical staff is also informed and able to evaluate every situation thanks to measurements of vitals collected in real time – body temperature, heartbeat, heart rate, etc. This feature is particularly important for elderly people living alone or in nursing homes for they could easily send their measurements to doctors or caregivers thus avoiding busy places like hospitals or other healthcare institutions.
